If you’re looking for a result with this or a similar layout, then no, not with the legacy / new form editor. If you wanted a fillable document, you’d have to look into integrations like Docusign or Pandadoc.
You could of course create a multi-step form that asks for the information displayed in your document, it would however be an overall different look and feel. Create and customize forms
Alternatively, you can of course always develop your own solution based on the HubSpot forms API.
