Do you have this setting enabled? Automatically populate contact properties from emails If not or if it was recently turned off, that would explain why contacts are missing first / last name.
There is no documentation on where else exactly the extension is pulling first and last name from – but could you also confirm that the contact is displaying in your inbox with a first and last name?
