What exactly is a call report, and why is it a form submission and not a logged call? If you want to report on activities (calls, meetings, etc.) I’d recommend using the appropriate tools in HubSpot.
Alternatively, Form Submissions is now a separate data source in the custom report builder (not sure if it’s still in BETA) so when you use that as primary source the report will show each submission and not de-duplicate contacts.
