Getting strange "ghost box" on forms when viewing on Safari

@Pmickey Does this happen when you use a HubSpot theme on your form? From the screenshot it looks like there may be external styling or scripts affecting this.

This is a Hubspot theme -- but altered a bit. We looked at the template and nothing there seems out of place. It's very odd.

We tried clearing everything out, nothing changed. This is only happening on Safari.

@Pmickey how did you alter it? HubSpot form's don't allow you to alter their themes. The reason I asked if you used a HubSpot form theme is because when a HubSpot form uses a form theme is renders inside of an iframe. So any external scripts/styling won't affect it.
If you enable a theme on a HubSpot form and this doesn't happen it shows that this is an external element influencing this.

The fact that this only happens in safari would lead me to one of a a few causes:

There's some sort of safari plugin/extension causing this (does this only happen for you on your safari or does this happen on other devices?
Something within your safari's cache is causing this, have you tried clearing your safari cache?
You have custom coding which is causing this to happen on safari .

I'd be leaning towards it being the last one to be honest. If it is the case, enabling a default HubSpot form theme would stop this, if you want to use your custom styling I'd say reach out to your web developer to find out what might be causing this conflict. Without seeing the actual website myself I can only guess on the issue though!
