Form looks strange on the page

Hello,

Sorry, it's probably a dumb newbie question, but I am struggling with adding a form to my page. When I am inserting the code for the form, it looks like this (like if it were added twice):

I have copied the code from Hubspot without changes.

What am I doing wrong?

A couple of clarification questions:

What CMS are you using?
Can you post a link to the live form on your site?

I am trying to add the form to the page in New Zenler page builder (a course hosting platform). They have an 'Add Custom HTML' block.

After a v quick look at the source code it looks like the form code is there twice but with two different styling variations hence why you have a full width version and a standard format version of the form. I'm not familiar with New Zenler so I'm not sure why that might be happening.

So, just to be sure - the code I get from Hubspot (shown below) is ok, but somehow it is added twice, right?
