&lt;P&gt;There isn't a template perse for the notes and activity logs on the contact record, but I think you could very well use a snippit for this purpose:&lt;/P&gt;

      <description>&lt;P&gt;Hi Hubspot,&lt;/P&gt;&lt;P&gt;&amp;nbsp;&lt;/P&gt;&lt;P&gt;How do l create a template for notes and activity log entries?&lt;/P&gt;&lt;P&gt;&amp;nbsp;&lt;/P&gt;&lt;P&gt;In our scenario we will have standard questions so that we need to log answers for, so l would like to create a dropdown for to propagate that information so staff can easily enter ansers without too much data entry&lt;/P&gt;&lt;P&gt;&amp;nbsp;&lt;/P&gt;&lt;P&gt;Thanks&lt;/P&gt;</description>

      <description>&lt;P&gt;Nearly every company has a template to prepare for calls and take notes on those calls. It seems like low-hanging fruit for there to be a way to log a call or meeting with a template.&amp;nbsp;&lt;/P&gt;</description>
