Hi @alexagatiss,

Happy to help here.

I want to have a request a quote form but see which page they submitted from.

With the HubSpot tracking code installed or when using HubSpot landing pages / website pages, HubSpot will store this automatically in the property "Recent conversion". The best way to check if your setup is correct is by simply creating the form, submitting it, then checking the value in "Recent conversion", looking up the form on the contact activity timeline and also checking the form submissions to see if they're being captured correctly. Most of the times, they are without further required setup.

I want it to be trackable in campaigns.

In the form editor, you can associate the form asset with a campaign. Once you do, form KPIs will be captured and shown inside of the campaign.

How does HubSpot track conversion page on forms?

Through the installed HubSpot tracking code: https://knowledge.hubspot.com/reports/install-the-hubspot-tracking-code – if you're referring to something else here, could you rephrase your question?

What is best practice for form setup so that conversion is tracked?

Using the HubSpot tracking code or native landing pages / website pages + associating forms and pages with campaigns + making sure on non-HubSpot CMS pages that the tracking code loads before the form embed to correctly capture referrer and source data.

Additionally, it's a good practice to use this form only once if it's important to track it individually. If it's only used once, you know that all conversion must come from that one page.

Let me know if you have any follow-up questions!

Best Practices for Including a Form on Your Website (With Page Tracking & Campaign Attribution)
If you're using HubSpot forms and want to track which page a form was submitted from (e.g. for a "Request a Quote" form), and tie it to a campaign, here are the best practices:

Use HubSpot Forms (not non-HubSpot or iFrames)

– Ensures native tracking and accurate attribution.

Track the Page of Submission

– HubSpot automatically tracks the submission URL if the tracking code is installed.
– For extra clarity, add a hidden field (e.g.,
Conversion Page
) and auto-fill it with Javascript:

document.querySelector('input[name="conversion_page"]').value = window.location.href;

Assign the Form to a Campaign

– After creating the form, go to Marketing > Campaigns, and add the form to the campaign to track submissions and influence.

Use the Same Form Across Pages

– HubSpot logs the submission source page even if the same form is embedded on multiple pages.

Ensure Tracking Code is Installed

<LI-WRAPPER>– HubSpot tracking code must be present on all pages where the form appears.
This setup ensures accurate conversion tracking, source attribution, and proper campaign reporting.</LI-WRAPPER>

@WebGuruz_Tech and @karstenkoehler , thank you for your thoughtful responses. I rebuilt the form in HubSpot (it was formerly a non-native form) and I can see that it is now tracking conversion pages. I currently only have one form but it is used on many different pages. I would like to associate the conversion page to different campaigns... if the form is submitted from a product page, I want it to associate it to that product campaign to track a new lead coming in. Similarly, if it comes in through a service page, I want it to track to the campaign associated to that service. Is this possible? My backup option is to just clone the form over and over again so that they are all separate and can be associated to different campaigns.
