&lt;P&gt;The short answer would be no, there is no file type that would behave like such a matrix, sadly.&lt;/P&gt;
&lt;P&gt;&amp;nbsp;&lt;/P&gt;
&lt;P&gt;If you want to capture this in a form, you would need one field per credential and one field per state.&lt;/P&gt;
&lt;P&gt;&amp;nbsp;&lt;/P&gt;
&lt;P&gt;This can be set up nicely with dependent form fields.&amp;nbsp;&lt;A href="https://knowledge.hubspot.com/forms/use-dependent-form-fields" target="_blank" rel="noopener"&gt;https://knowledge.hubspot.com/forms/use-dependent-form-fields&lt;/A&gt;&lt;BR /&gt;&lt;BR /&gt;&lt;/P&gt;
&lt;P&gt;I'd recommend starting with a field "Number of credentials" (dropdown!), then setting up the dependent fields. If 1 is selected, show credential 1 and state 1. If 2 is selected, show credential 1, state 1, credential 2, state 2 etc&lt;/P&gt;
&lt;P&gt;&amp;nbsp;&lt;/P&gt;
&lt;P&gt;Theoretically you could then copy the information into custom objects for easier management. This would require workflows and ideally an Enterprise subscription.&lt;/P&gt;

      <description>&lt;P&gt;We want to create a form or way to capture for a contact's professional credential (MD, BCPA, LISCW, RN), the state (or states) where it's valid, and the expiration date of the credential.&amp;nbsp;&lt;/P&gt;&lt;P&gt;&amp;nbsp;&lt;/P&gt;&lt;P&gt;Is there anyway to do a matrix type question where if you pick that you are an RN, you then select the state (or states) and enter an expiration date? Is there a way to store it in the database like that also?&lt;/P&gt;</description>
