Token Refresh API not working

I'm trying to refresh this user's token, but something seems off with HS API. Sometimes it works, and other times it doesn't. I made the same request multiple times and it occasionally returns the error "MISMATCH_REFRESH_TOKEN_CLIENT" while other times it works fine, any ideas on what's going on?
