Two-column form now displaying as one-column

We have a form which was displaying as a 2-column form. Now it's displaying as 1-column (see attached screenshot)

I've checked in different browsers, community pages and searched web, all Hubspot, etc, and still can't see a solution. In fact, I found it maybe an issue with Hubspot forms here (not sure this helps tho) > https://community.hubspot.com/t5/Blog-Website-Page-Publishing/Multi-column-form-width-problem/td-p/762575

Anyone know how do we have the form return to 2-columns?

It was working fine when we launched the site on Nov 28 last year. Since then, no changes have been made to that page that I'm aware. Can anyone spot what might have changed to cause this?

Thanks for your reply Anton. The form displays when pressing Request A Callback button on 4th step of Calculator here > https://www.homewise.co.uk/calculator > Attached screenshot shows what it looks like.

1-column, should be 2-column form
