NetSuite App - Manually Create Sales Order & Field Options Not Mapping

Our HubSpot app is integrated with the NetSuite using the app built by HubSpot. We are trying to manually create a sales order from a HubSpot deal record. However, the options for the required Location field are not loading. There's a spinning wheel next to the field. Eventually a red "something unexpected went wrong" banner will display. We have not been able to create a sales order because of this missing requirement.

As a side note, we've tried to use workflows to create the sales order using the Netsuite app's workflow action. These fields were mapping properly but we ran into an issue of HubSpot creating duplicate sales orders in NetSuite.
